The Health Information Management (HIM) Department is responsible for maintaining medical records. We perform assembly, analysis, transcription, coding, scanning and release of information.

Obtaining Copies of your Medical Record
You may click on the "Authorization for Use and Disclosure of Patient Information" form found below. Please complete the form and fax it to our department to obtain copies of your medical records.

Cost associated with release of information
Each year the state of Ohio releases information on what hospitals can charge to provide copies of medical records. It is best to contact our department and we will be happy to provide you with this information. There is no charge for records obtained for continuation of care.
Birth Certificates
The Bellevue Hospital does not have copies of birth certificates at the hospital. If you would like a copy of your birth certificate, go to the Ohio Department of Health’s website to review information on how to obtain a birth certificate or other vital statistics.
